You will find a separate page to download the New Client Form. Please fill out the form and send it, along with any previous medical records (vaccine history, diagnostics, etc), to our clinic within 72 hours of your first appointment with us. You can fax it to 602-787-0505 or email to recordsflf@gmail.com. Receipt of these items will serve as your confirmation of your scheduled appointment.
As a new client with our practice, we ask that you please be patient with our Doctors and staff as we try to accommodate the needs of all of our clients, old and new.
